How it Works.

You can download the form or you may contact the

DEM office at 859-258-3784 to receive a form by fax or U.S. mail.

Residents can now complete a Special Needs Survey form, available on the Department of Emergency Management's website at www.lexingtonky.gov/dem.

The form will allow residents to describe any special needs they might have and will provide emergency response personnel quicker access to information about people who might need special assistance during an emergency.

The information will only be used for public safety during a disaster or emergency.

Having a registry that pre-identifies residents of our community who might need special assistance during an emergency will allow Lexington to respond quicker and more efficiently to the needs of the community.
