The Ohio Medical Board has permanently suspended the license of a Kentucky anesthesiologist.
Doctor Lee Balaklaw made an Alford plea in May to 10 misdemeanor counts of theft by deception under $300.
He was sentenced to 12 months in jail, the sentence was suspended for two years if he pays a more than $140,000 restitution.
In October 2011, a Lawrence County grand jury indicted Balaklaw on 20 counts of Medicaid fraud.
The pain clinics owned by the anesthesiologist are now closed.
